To honor and recognize a physical therapist who, in the past year, has made significant contributions and demonstrated commitment toward the betterment of the profession in the areas of practice, public relations, legislation, education or membership.
Eligibility Requirements
Nominees must be current members of the association and have been so for at least three years, have a clean disciplinary record with the state board of physical therapy,  have not been previously awarded this within the past five years, and have a minimum of three years of experience as a physical therapist.


Selection Criteria
Candidate has made exceptional contribution to the association in an elected or appointed position at the chapter or national level of the association this past year.
  1. Professional Development: The nominee has demonstrated a commitment to their own professional development and continued learning, through attending conferences, courses, and other educational opportunities. Additionally, the nominee has contributed to the professional development of the physical therapy profession overall, through sharing knowledge and expertise with other professionals, mentoring students, and participating in professional organizations and activities.
  2. Patient Care: The nominee has demonstrated exceptional patient care and outcomes, showing a commitment to evidence-based practice and clinical excellence. This includes utilizing the latest research and techniques to provide the best possible care to their patients, as well as consistently achieving positive patient outcomes. In addition, the nominee has gone above and beyond in their care of patients, whether by taking extra time to answer questions, providing emotional support, or finding creative solutions to challenges.
  3. Advocacy: The nominee has been an advocate for the profession, actively participating in legislative efforts to improve access to care and advance the profession. This may include advocating for fair reimbursement rates and increased access to care for patients, meeting with legislators, or working with professional organizations to develop and promote policy initiatives. Examples of how the nominee could be engaged in advocacy efforts are as follows:

Legislative Efforts: The nominee has been actively involved in legislative efforts to advance the profession, such as advocating for fair reimbursement rates and increased access to care for patients. This may include participating in advocacy campaigns, meeting with legislators, or working with professional organizations to develop and promote policy initiatives.
  5. Community Outreach: The nominee has engaged in community outreach initiatives to raise awareness about the benefits of physical therapy and promote the profession. This may include participating in health fairs, speaking at local events, or providing educational materials to the public.
  6. Public Relations: The nominee has been involved in public relations efforts to promote the profession and enhance its visibility. This may include working with local media outlets to feature stories about physical therapy, creating and sharing social media content, or developing marketing materials to promote the benefits of physical therapy.
  7. Collaboration: The nominee has shown exceptional collaboration skills, working closely with other healthcare professionals and organizations to improve patient care and outcomes. This includes working as part of an interdisciplinary team, sharing knowledge and expertise with other professionals, and serving as a mentor or teacher to students and colleagues.
  8. Leadership: The nominee has demonstrated leadership qualities and has been an influential figure within the profession, inspiring and mentoring others. This includes serving as a role model for other providers, exhibiting a commitment to ethical and professional behavior, and showing a willingness to take on new challenges and opportunities.

  • Current resume or curriculum vitae of nominee
  • A letter of nomination that specifically focuses on the strengths of the nominee in relation to the award criteria, not to exceed 3 pages
  • Letters of support are welcome but not required from individuals who can specifically address one or more of the criteria for selection.
